NCD is a PPO plan that utilizes the MaximumCare PPO networks, including Careington, Connection Dental, and DenteMax. Our plans cover 100%* for covered preventative services, 80%* for covered basic services, and 50%* for covered major services after the 12-month waiting period. Some members may have the 12-month waiting period waived with qualifying proof of prior coverage.

Precertification is not required; however, it is recommended for services expected to exceed $1000.00.
